He is survived by: his wife Jennifer Slywczak (Sibiga) of Kinsman; his children, Daphne Rose Slywczak of Kinsman, Rachel Irene Slywczak of Kinsman and Jeremy John Slywczak of Kinsman; and his siblings, Edward Slywczak of Kinsman and Barbara Slywczak of Kinsman. He is also survived by many loving sister-in-laws and brother-in-laws, nieces and nephews.
