He was predeceased by: his parents, Francis C. Hook and Nettie E. Hook; and his son Jason Michael Hook. He is survived by: his wife Patty Hook; his daughter Kasey Shaffer (Chris); and his grandchildren, Keirstyn and Brayden Shaffer.
