We are sad to announce that on October 24, 2023, at the age of 76, Thomas Ray Hazlett of Franklin, Ohio, born in Mount Vernon, Indiana passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He was predeceased by: his parents, Kent Hazlett and Faye Hazlett (Dunaway). He is survived by: his wife Janice Hazlett; his daughters, Heidi Siens (Kurt) and Heather Faulkner (Jason); his grandchildren, Kyle, Blake Fella, Mackenzie and Olivia Faulkner; his great grandchildren, Joel and Lily; and his sisters, Linda Miller (Tom) of Franklin and Carol McNally (William) of Orlando, Florida. He is also survived by numerous nieces and nephews.
In lieu of flowers the Hazlett family has requested that memorial donations in Tom's name go to https://agentorangerecord.com to aid with veterans and their families that were exposed to Agent Orange.
