He was predeceased by: his parents, Roger Phillips and Beatrice Phillips (Waller). He is survived by: his children, Michael Phillips of Fostoria, Christy Hedrick (Rod) of Fostoria, Brent Davis of Findlay and Bryce Pitney of Findlay; his grandchildren, Alicia Garcia (Hugo) and David Delacruz (Amber); his great grandchildren, Isaac, Tristan and Samantha Garcia; and his siblings, Deborah Phillips, Gary Phillips and Andy Phillips (Deb).

Memorial contributions may be made in Clark's honor to the American Cancer Society or to the American Heart Association.
