He is survived by: his wife Kathryn L. Temple; his children, Jessica Tink (Levi) of Pleasant Lake, Jennifer Cook of Edon and Matthew Cook of Lake Seneca, Ohio; his grandchildren, Ana, Levi, Jr., Destiny and Savanna Tink; and his siblings, Kevin Cook (Jodi) of Salem, Indiana and Raquel Link (Lloyd) of Craigville, Indiana.
