He was predeceased by: his parents, Leo Cook and Adelia Cook (Fitzgerald); his siblings, Mary Benson and Robert Cook; and his daughter Elizabeth Cook Thompson.
He is survived by: his wife Jane; his son James F. Cook (Renee); his son-in-law Dr. John Thompson; his grandchildren, Kyle, Zoe Thompson, Elizabeth, Adeline and Penelope Cook; and his sister Margaret Twohey.
