He was predeceased by: his parents, Ron Tollafield and Alice Tollafield (Evans); and his sister-in-law Lynne Tollafield. He is survived by: his brothers, Bob and Mark Tollafield (Karen); his nephews, Stephen (Eric Anderson), Andy (Abby) and Jeff Tollafield (Nicole); his niece Amy Tollafield (Luke Carpenter); his great-nieces, Lily and Kate Tollafield; and his great-nephew Bennett Tollafield.

A private family burial is planned at Ohio Western Reserve National Cemetery however the family suggests that donations in his memory can be made to the Medina County SPCA, 8790 Guilford Road, Seville, OH 44273.
