He was predeceased by: his parents, Richard D. Polen and Margaret Proehl Polen; and his siblings, Richard "Dick" Jr., Lance, Keith, Jack Polen, Kevin Polen and Tamara Polen.
He is survived by: his daughter Serena S. Polen (John Miller); his granddaughter Savannah Knece; his great grandchild Braiden Grover-Pollard; his stepdaughter Sandra Horsley; and his siblings, Christie Bethel (David), Susan Spetnagel (Dennis), Jill Adams and Mark Polen of Chillicothe. He is also survived by several nieces and nephews.
