He was predeceased by: his parents, Samuel Forsythe and Freeda Forsythe (Steele); his wife Judy Forsythe; his son Michael Forsythe; and his siblings, Robert Forsythe (Red), William Forsythe, Richard Karl Moore, Betty Pepe and Alberta Bates.
He is survived by: his daughter Melissa Kahl (William); his siblings, Samuel Forsythe-Danford of Florida and Nancy Kirby of Colorado; his grandchildren, Alexis Sprague (Joshua), Justin Kahl and Zachary Kahl; and his great grandchildren, Emmett and Rennen Sprague. He is also survived by several nephews, nieces and cousins.
