He was predeceased by: his parents, Milton Cook and Garnet Cook (Perrigo); and his granddaughter Courtney. He is survived by: his wife Linda Slack; his children, Shelly Walther (Chris) of Sprakers, NY, Patrick Cook, Chris Cook, Scott Gay, Jason Gay (Jodi) and Andy Cook; his grandchildren, Amanda (Alando), Nicholas (Cerina), Emily, Luke, Matthew, Stephanie, Skyler, Mackenzie, Nathan (Amanda), Megan (Cody), Katie, Rebecca and Johann; and his great grandchildren, Azlyn, Zoey, Everleigh, Levi, Adalyn and Leo. He is also survived by several nieces, nephews and cousins.
In accordance with the National Funeral Directors Association and the CDC, masks must be worn and proper social distancing practiced.
