He was predeceased by: his parents, John Polak and Agnes Polak; his siblings, Joseph, Chet and Mary; and his son John Polak.
He is survived by: his children, Thomas Polak, Rosaleen Polak (Cookie), Robert Polak, Paul Polak and Lisa Polak. He is also survived by great grandchildren.
