He was predeceased by: his parents, Joseph Forrest and Josephine Counihan; and his siblings, Gregory, Nancy and Thomas.

He is survived by: his wife Theresa (Lesya); his grandson Jeremy; his nieces and nephews, Amy, Mercedes, Tommy, Samantha, Ava and Tatum; and his pet Couni. He is also survived by his mates, neighbors, friends.
