He was predeceased by: his parents, Kenneth Cook and Harriet Cook (Coughtry). He is survived by: his wife Susan Yager Cook; his children, Christine, Leslie (Robert Westphal) and Stephen (Christine Marmo); his granddaughters, Elisabeth Johnson (Daniel) and Roberta Sutton (Chris Kelly); and his great grandchildren, Franny, Finbar and Jem.
