He is survived by: his wife Pamela; his children, Richard Michael Siebel (Lori) of North Carolina and Elisabeth Ann Given of Lake Grove, NY; his grandchildren, Brett Evan Siebel (Jenna), Bailey Renee Siebel of North Carolina and Jillian Taylor Given of Lake Grove, NY.; his siblings, Walter, Robert and Ellen Leiss; and his sister-in-law Dorothy Taylor.

In lieu of flowers the family suggests donations be made in his name to the American Diabetes Society.
