He was predeceased by: his parents, John E. Falkowski and Jean Falkowski Sr. (Pawlick); and his daughter Dana Falkowski. He is survived by: his wife Cynthia Falkowski (Kowalski); his sons, John (Dawn Falkowski) and James Kowalski; and his grandchildren, Quentin, Brittany Falkowski and Sam Kowalski.
