He was predeceased by: his father Ralph Hazen; and his brother Timothy Hazen. He is survived by: his mother Barbara Hazen; his wife Kathy; his children, Alex Hazen (Jennifer), Katelyn Farron (Brandon) and William Hazen (Sam); his siblings, Vicki Lehner (Don), Michelle McGurk (Joe) and Randy Hazen (Tina); and his grandchildren, Avery Farron, Grayson Hazen and Jack Hazen. He is also survived by many nieces, nephews, cousins and extended family.
