He was predeceased by: his parents, Paul Vlasak and Ida Vlasak; and his brother Thomas Vlasak. He is survived by: his siblings, Gary (Angie Vlasak), D. Michael Vlasak, Terry (Patricia Vlasak) and Karen Sherman. He is also survived by several nieces, nephews and cousins.
