He is survived by: his wife Brenda Middendorf; his children, Brian Doty, Lyndell (John Miller), Joe (Elizabeth Doty), Anthony (Tiffany Middendorf), Mark (Julie Middendorf) and Nicole Middendorf; his grandchildren, Hope, Nathan, Joey, Julia, Grace, Stella, Ephriam, Mark, Lilly of Ivy, AJ, Ty, Alex, Lexi, Natalie and Ralph; and his siblings, Gerald "Bill" (Kathleen Middendorf), Elizabeth (Gerald Pencek), Robert Middendorf, Wayne (Cindy Middendorf), Dennis (JoAnn Middendorf) and Matthew Middendorf. He is also survived by numerous nieces, nephews and cousins.
