He was predeceased by: his brothers, Ned and Clayton. He is survived by: his wife Lillian Cushman; his children, Thomas Richard Cushman (Carol), Donna Jean Murphy (Kevin) and David Allen Cushman (Gail); and his grandchildren, Matthew (Brittney) and Kristin. He is also survived by several nieces, nephews and dear friends.
In lieu of flowers, contributions may be made to St. Jude Children's Hospital in his memory.
