He was predeceased by: his wife Janice Bosiacki; his parents, Casimer Bosiacki and Ida Bosiacki; and his siblings, Richard and Joanne Bosiacki.
He is survived by: his sons, Michael and Kevin Bosiacki (Karen); his grandchildren, Jack and Lily Bosiacki; and his brothers, Joseph (Donna) and Edward (Christine). He is also survived by nieces and nephews.
In lieu of flowers, memorial donations may be made in Tom's name to the Buffalo City Mission and St. Jude's Children's Research Hospital.
