He was predeceased by: his parents, Michael J. Cook and Elizabeth M. Cook (Abbate); and his brother David Cook. He is survived by: his wife Dorothy A. Cook (Benoit); his sisters, Marybeth Nadell (late Ave), Barbara Kaperka (John), Michele MacCue (late Thomas) and Donna Iannacito (Antonio); and his sister-in-law Patricia Cook. He is also survived by nieces and nephews.
