He was predeceased by: his brother Danny; and his nephew Dustin. He is survived by: his wife Cindy Phillips; his children, Jerry Jr., Nate and Sami; his stepchildren, Jessica and Nick; his granddaughter Saige; and his siblings, Lisa and David. He is also survived by several nieces, nephews, aunts and uncles.
