He is survived by: his parents, Forrest William Thomas and Melissa Ann Decker; his children, Madilyn, Zaydin and Camden; his grandfather Forrest Thomas Sr; his niece Reighla; and his nephews, Brendyn and Isaiah. He is also survived by three brothers and a sister in law.
