He was predeceased by: his parents, Thomas Cusack and Agnes Cusack (Griffin); his wife Ann; and his sister Marilyn. He is survived by: his sons, Thomas, Robert and William; his daughters-in-law, Mary and Marianne; his grandsons, Thomas IV and Nicholas; and his brothers, Robert of Connecticut and Bill of Ohio. He is also survived by his nieces, nephews, cousins, other relatives and friends.
