He was predeceased by: his parents, Frank L. Middlebrook and E. Irene Middlebrook; and his brother Wayne Middlebrook. He is survived by: his wife Bonnie; his children, Jamie Ellis (Gene) and Toby Middlebrook (Kristi); his grandchildren, Jaedyn, Klowee Middlebrook, Jazmine and Iris Ellis; and his pet Rusty. He is also survived by many nieces and nephews.
