She was predeceased by: her parents, Vivian Torres and Jose R. Collazo; and her brothers, Roger Serrano and Kevin Negron. She is survived by: her fiancé Joseph M. Purificato; her sons, Rafael Díaz, III and Cameron Purificato; her grandchildren, Rafael Díaz IV and Amarius Díaz; her stepchildren, Joseph and Adriana Purificato; her siblings, Edwin N. Burgos, Damarie Burgos (Julio), Joseph Collazo (Beatriz) and Joel Collazo; her aunt Millie Collazo-Howard (Larry); and her mother-in-law Margaret Maines (Gilbert). She is also survived by nieces, nephews, cousins, family, and close friends.

In lieu of flowers, donations to Joseph Purificato are welcomed in memory of Chastity.
