He was predeceased by: his father John; his brothers, Glen and Jeff; and his grandmother Gladys Perkins. He is survived by: his wife Lisa; his mother Elizabeth Eyler; his children, Alicia Jackson, William Perkins and Evan Perkins; his grandchildren, Jacob Perkins and Savanna Crowly; his siblings, Dean Eyler, Patricia Halko (Bob), Cindy Eyler and Susan Eyler; his siblings-in-law, Linda Foster, Noreen Perkins, Henry Moyer (Diana) and Steven Moyer (Sando); his aunt Carol Mathias (Lee); his uncle Douglas Perkins (Bernadette); and his mother-in-law Sylvia Moyer.
