She was predeceased by: her husband John Fornazor; her parents, Peter Ceselka and Helen Ceselka; her sister Mary Zizik (Frank); and her sister-in-law Mary Diamond.
She is survived by: her children, Susan Dugan (Jack) and John Fornazor (Peggy); her grandchildren, Johnny Fornazor (Christina) and Lauren McMackin (Shawn); her great grandchildren, Johnny Daniel Fornazor and Shawn Patrick McMackin; and her sister-in-law Rita Concilla. She is also survived by nieces and nephews.
In lieu of flowers, donations can be made to the Assumption of the Holy Virgin Orthodox Church.
