He was predeceased by: his parents, Michael Hooks and Annie Hooks (Kenny). He is survived by: his wife Mary Hooks (Andersen); his children, Anne Olsen (Hooks) and Anthony Michael Hooks (Rachel Hooks); and his granddaughters, Julianna Hooks, Danielle Hooks and Fiona Olsen. He is also survived by nieces and nephews.
