She is survived by: her children, Richard Forman (Debbie), Gary Forman (Donna), William Forman, Larry Forman, Carol DuBois, Linda Hanson and Ellen Rue (Ron); her siblings, James Costello, Joseph Costello, William Costello, Patricia Meyers, Marjorie Heuser and Joyce Montesano; and her siblings-in-law, Norma Scott, Elaine and Boyd Griffon. She is also survived by ten grandchildren; and nine great grandchildren.
