He was predeceased by: his siblings, Anna Mae, Dolores, Lorraine, Joseph, James and Edward; and his son-in-law Michael Hazzard.
He is survived by: his former wife Anna Rokuskie; his wife Helen McDade (Tyminski); his children, Lisa Hazzard of Chanhassen, MN and John; his daughter-in-law Jill McDade of Eagan, MN; his siblings, Gerard McDade and Joanne Rile; his grandchildren, Mazie (Justin), Johnny and Serena; his great grandchildren, Phoebe and Adrian; and his stepdaughter Nicole. He is also survived by nieces, nephews, cousins and friends.
