He was predeceased by: his wife Marie Louise Schmidt Slack. He is survived by: his sons, Frederick Philip Slack (Pat Henriques) and Philip Joseph; and his brother Charles Slack (Sue). He is also survived by his six grandchildren; and his eleven great grandchildren.
