He was predeceased by: his parents, Raymond Cudaback and Nellie Cudaback (Thomas); and his brothers, Eugene and Ronald. He is survived by: his wife Caroline Havlovic of Schuyler; his sons, Douglas Cudaback (Tammy) of Beatrice and Jeffrey Cudaback of Bellwood; his granddaughters, Brittany, Abby, Jess, Josie and Jerri; and his great grandchild Calvin Cudaback. He is also survived by many nieces and nephews.
