He was predeceased by: his parents, Guy Formy Duval and Reba Stanley Formy Duval; his sister Marvalene Tison; and his nephew Timothy Tison. He is survived by: his siblings, Nelda Tison of Nakina, Joanne Thompson of Whiteville and Arnold Formy Duval (Hilda) of Nakina; his nieces, Robin, Lynette, Karla and Gina; and his nephew Trevor. He is also survived by three great nephews, two great nieces.
