He is survived by: his son Tim Blake (Lenena) of West End; his stepdaughter Melissa McIntosh (John) of Crossville, TN; his caregiver Sharon Parker of Jackson Springs; his siblings, Jeremy Keith Blake (Angie) of Beulaville, Anthony Wesley Blake (Tiffany) of Seagrove, Kenny Andrew Blake (Melissa) of Guam, Terry Fredrick Blake of Carthage, Pamela Elizabeth McIntosh (James) of Carthage, Sharon Louise Blake of West End and Jennifer Kelly Thomas (Kenneth) of West End; and his grandchildren, Hunter Blake, Adley Johnson, Madison Reano (Aaron), Jody McIntosh, Natalie McIntosh, Reagan McIntosh, Cynthia Diane McIntosh and Rick.
