He was predeceased by: his parents, Lenoir Bruce Cook and Flossie Powell Cook; his wife Ada Davis Cook; his sons, Anthony Cook and Sammy Frye; and his siblings, Luther, Howard, Hubert Cook, Beulah Parlier and Lola Dale.
He is survived by: his sons, David Cook of Cape Coral, FL and Michael Cook (Annette) of Granite Falls; his sisters, Evelyn Phillips of Granite Falls and Doris Pruitt of Hudson; his grandchildren, Michele Mizen (Gunther) of Cape Coral, FL, Garrett Cook of Cape Coral, FL, Ashley Cook of Cape Coral, FL, Brittany Baggaley (Richard) of Granite Falls, Brandon Kirby (Amanda) of Texas, Kristi Davis (Mike) of Vale, NC and David Frye; and his great grandchildren, Ariana Frye, Grace Baggaley, Ellie Baggaley, Robbie Kirby, Isabella Kirby, Kristen Davis, Brandie Davis, Kellan Mizen and Zoey Mizen.
