He was predeceased by: his parents, Perry Cook and Alcie Cook; his wife Gerri Conard Cook; his daughter Melita Cook Sorrells; his granddaughter Tamsen Powers Burleson; and his siblings, Earl Cook, Ruby Gibson, Edith Cook, Martha Mease and Marie Steppleton.
He is survived by: his children, Vicki Cook Powers (Tom), Guy Dean Cook (Karen) and Sherri Cook Burch; and his grandchildren, Caleb Burch, Tori Powers Hughes and Tessa Powers Hull. He is also survived by five great-grandchildren.
