He was predeceased by: his parents, Mike Smith and Johnetta Crockett; and his sister Carrie Smith. He is survived by: his wife Kimberly J. Crockett; his children, Sydney Haven McCall, John Michael Crockett Jr. and Ryan Cole Crockett; his sisters, Jennifer Cline (Russ) of Wilmington and Tori Smith of Sullivan, IN; his grandmother Jenny Smith of Vincennes, IN; and his nephew Stephen Przybylak of Wilmington.
