He was predeceased by: his parents, Percy Smith and Lucille Newby Smith; and his siblings, Harold Smith, Beona England and Polly Bowers.
He is survived by: his spouse Shirley Parrish; his daughters, Arrow Blackwell (Todd) of High Point and Dowa Proctor (Dale) of Archdale; his sister Cynthia Lynch (Gary) of Kernersville; and his grandchildren, Mavrick Blackwell, Koty Proctor and Kory Proctor.
In lieu of flowers, memorials may be directed to two locations: Chuck Clements Scholarship Award for Officials' children at 212 Lexington Pl.
