She is survived by: her children, Terrell "Turk" Cook (Kimberly) and Lori "Cissie" Forsythe (Denny); her brothers, William "Bill" Mullins of Pell City, AL and Ben Mullins (Shirley) of Vancleave, MS; her grandchildren, Tyler Cook (Kendall), Trent Cook (Alyssa), Taylor Cook, Trey Forsythe and Logan Forsythe; and her great grandchildren, Kylee Cook, Kynsleigh Cook, Kyler Cook and Lucy Leigh Cook. She is also survived by numerous nieces, nephews, and other family and friends.
