He was predeceased by: his parents, Donald Perry and Carolyn Marie Havard Cook; and his son Justin Michael Eubanks. He is survived by: his wife Sonya Cook; his children, Michillie Pecue (Ray), Clent Cook, Mary Havard and Cody Pierce; and his grandchildren, Donalea Minchew (Josh), Codie Whatley, Rayland Pecue, Koley Havard, Mattie Havard, Harley Havard, Layla Havard, Rosanna Havard and Hannah Parnell. He is also survived by 6 great grandchildren; a host of aunts, uncles, cousins, other relatives and friends.
