He was predeceased by: his grandparents, Barbara Middleton and Leroy Corlett. He is survived by: his parents, Wesley Corlett and Annette Corlett; his wife Ashley; his daughters, Annalee, Jenivee and Aveline; his siblings, Diane Mozier (Lukas) and Daniel Corlett (Jennifer); and his grandparents, George Middleton and Helen Corlett. He is also survived by many other family members and friends.
In lieu of flowers David wanted a fund to be set up for his daughters.
