He is survived by: his wife Theresa Bax of Jonesburg, MO; his daughters, Jeanette Bax-Kurtz (Mark Kurtz), Carol Whayne, Mary Jane Petralli (Tony) and Karen Murphy; his grandchildren, Emily Boner (Billy), Mark John Kurtz, Jr., Lauren Merchant, Andria Petralli, Jaimie Kurtz, Brittany Merchant, Hannah Murphy and Brendan Murphy; his great grandchildren, Aubrie Boner, Naomi Boner and Everett Dare; his siblings, Robert Bax (Sharon), Evelyn Loethem, Rosanna Bisges (Gene) and Rosetta Sommers (Eddie); and his sister-in-law Ramona Bax.
