He was predeceased by: his parents, Jake and Mary; and his sister Glenna Mae Parks. He is survived by: his significant other Kat Simpson Mendoza; his son Jerry Cook, Jr.; his wife Mikki Cook; his daughter Tracy Wiseman (Roy); his siblings, Naomi Renck (Jae), Gail Cook, Bob Cook, Joe Cook, Robert Cook (Lucille) and Jim Cook; and also Scotty Cook (Christy). He is also survived by 5 grandchildren; and 1 great grandchild.
