He was predeceased by: his father Winfred Harold. He is survived by: his mother Brenda Kaye Burkheart Cook of Lewistown; his children, Brayton and Jacie Cook of Leonard, MO; and his siblings, Jason Cook (Amanda) of Shelbyville, MO, Wendy Kirby of Shelbyville, MO, Cindy Cook (Patrick Nampel) of Marshall, WI and Mindy Cunningham (Michael) of Moberly, MO. He is also survived by several nieces, nephews, aunts and uncles.
