J and Jeremy Midcap; his great grandchildren, Alyssa, Suzy, Grayson Swearingin, Landyn, Keegan, Dallas, Christian and Noah Midcap; his sister-in-law Sarah Midcap of Sistersville, WV.; his son Forrest A. Midcap, Jr.; and his brother Jim Midcap.
