He was predeceased by: his parents, Hamilton Eugene and Sharron Cook (Keller); and his brother Michael Cook. He is survived by: his sons, Harley Cook of Jacksonville, Austin Cook of Belle and Jessie Cook of Freeburg; his brother Jerry Cook of Belle; his nephews, Cody Dennis and Michael Cook; and his great-niece Marla Cook. He is also survived by other extended family members and friends.
