He is survived by: his close friend Kathy Sweeney; his parents, Cyril Kubista and Jean Kubista; his siblings, Norma Fogarty (Mike) and Gary Kubista (Lori); his nephews, Brian Fogarty (Sonja Hadler), Dan Kubista (Jennifer) and Bradley Fogarty (Dana); his niece Melissa Kubista (Cory Schmitz); his great-nieces and great-nephews, Tucker Fogarty, Collin Fogarty, Dylan Kubista, Kolten Hoffmann, Chase Hoffmann, Kooper Schmitz, Mikenna Fogarty, Ava Fogarty and Kaylee Fogarty (Kathy Sweeney); and also, Kim and Joe Sweeney.
