He was predeceased by: his parents, Phil Clark and Shirley Clark; and his daughter Abigail. He is survived by: his wife Monica; his sons, Andrew and Matthew; his stepmother Ann Clark; his siblings, David Clark (Sarah) and Marguerite Clark; his aunt Linda Schwartz; his mother-in-law Vida Lanari; and his cousins, Sara Flowers and Brian Holmes.
In lieu of flowers, memorials will be distributed at a later date to causes important to Sam.
