He is survived by: his wife Kathy Reneé Theisen; his children, Samantha Rose Theisen, Michael David Theisen and Jonathan Patrick Theisen; his father Dave Theisen (DeVada); and his parents-in-law, John Ouellette (Pam) and Cheryl Johnson (Norman). He is also survived by 37 nieces and nephews; 18 great nieces and nephews; other family and many friends.
